Radionuclides therapy (RNT) uses radionuclides as agents mostly for cancer therapy. When compared with other systemic cancer treatments, RNT has shown efficacy with minimal toxicity. Moreover, radiobiology needs to be further to be explored, and It can lead to updating for systemic radionuclide therapy strategy. Radionuclides and radiobiology concepts are the importance of basic concepts of ionizing radiation. Traditional radiation biology concepts stated that any immature, undifferentiated, and actively dividing cells are radiosensitive. The paradigm shift in understanding radiation’s effect on biological systems from DNA-centric dogma to radiation effects involving multiple cellular targets. The evolution of radiobiological concepts also needs to be understood in radionuclides therapy (RNT) application. Radionuclides are conjugate with various peptide receptors, the monoclonal antibody, or vectors with affinity to specific elements have been a promising therapeutic strategy that makes radiation delivery to tumor sites optimally. This review focus on radiation biology at the molecular level and radionuclides’ therapeutic application concepts.
